Claims Settlement

To lodge your claims on a policy has been significantly simplified. All you need to do is send a mail to Claims@wapic.com and your claims will be attended. Alternatively, you can call our dedicated claims hotlines:

To facilitate the Claims Settlement Process, the underlisted are required for specific classes of claims:
 

The underlisted are required:
  • Duly completed claim form
    • All Risk), claim form
    • Burglary, claim form
    • Goods-in-transit claims form
  • Statement of claim (detailing the lost or damaged items with value),
  • Police report
  • Fire Insurance
  • Duly completed claim form,
  • Statement of claim (detailing the lost or damaged items with value).

  • Duly completed claim form,
  • Statement of claim (detailing how much was stolen and by who),
  • Name of the culprit(s),
  • The culprits' letter (s) of employment,
  • Police report.

The underlisted are required for claims under this policy:
  • Executed claims notification letter:
  • Original copies of the following substantiating documents for sighting:
  • Medical Certificate of Cause of Death,
  • Death Certificate,
  • Burial Certificate,
  • Coroner's report (if death is by mysterious circumstances),
  • Police Report (if death is by road traffic accident).
* The substantiating documents will be returned to you after sighting by a Wapic Representative
 
For the homeowner insurance, the underlisted are required to lodge your claim:
  • Duly completed claim form
  • Witness statement/Security report (if any)
  • Purchase receipts/Invoice of stolen items
  • Quotation for the repairs of damage parts a certified building contractor
  • Police Report (this is applicable if the claim is a Burglary case)
  • Fire brigade report (in fire claims)
  • Photograph of damaged properties or premises
This applicable under Wapic Cash, Wapic Val, Wapic Trust, Wapic Gold and Wapic Wealth Accumulator and other investment links products as stated in the terms and conditions of the policy:
  • Completed loan application form or a letter of intention to take a loan (See policy terms and conditions)
  • Original policy document.
For minor incidents involving your vehicle, the documentation for claims lodgement is as follows:
  • Duly completed claim form
  • Repairs estimate from a certified mechanics
However, if the vehicle is damaged beyond repairs of written off, the documentation required is as follows:
  • Original purchase invoice
  • Original vehicle license
  • Original certificate of insurance
  • Ignition and other lock keys
  • Photograph of the damaged vehicle.

  • Complete a withdrawal form or send a letter of intention to part-liquidate.

Injury Cases

  • Duly completed Claims Form (Group Personal Accident/Personal Accident/Workmen Compensation)
  • Medical report
  • Pay slips for 3-months preceding injury

Death Cases

  • Duly completed Claims Form
  • Medical report
  • Pay slips for 3-months preceding injury
  • Burial certificate
  • Certificate of cause of death.

Death

In the event of the untimely passing of a love one, the underlisted are required:
  • Duly completed Claims Form
  • Original Policy Document
  • Original copies of substantiating documents as follows* :
  • Medical Certificate of Cause of Death,
  • Death Certificate,
  • Burial Certificate,
  • Coroner's report (if death is by mysterious circumstances),
  • Police Report (if death is by road traffic accident).

* The substantiating documents will be returned to you after sighting by a Wapic Representative

In addition to the foregoing, the following forms must also be executed and submitted to our Claims department:
  • Proof of claim on death (to be completed by the deceased Personal Medical Attendant)
  • Declaration to establish identity in case of deceased policyholder (to be completed by someone other than a relative or the Medical Attendant or person interested in the assurance),
  • Proof of claim on death (to be completed by the claimant).
This applicable under Wapic Cash, Wapic Val, Wapic Trust, Wapic Gold and Wapic Wealth Accumulator and other investment links products as stated in the terms and conditions of the policy:
  • Complete a termination form or send a letter of intention to disengage from the scheme,
  • Submit original policy document.
